The Northwest Florida Physicians Group Process

 

  • Be evaluated for your qualifying condition and bring these records to your visit.
  • Call our office to schedule an appointment and state your qualifying condition.
  • Our staff will mail or email you a copy of our brief new patient paperwork when you schedule. 
  • On the day of your appointment, you will need to arrive twenty minutes early.
  • What to bring with you:
  •  Medical records that support the diagnosis of your qualifying condition.
  •  A list of your current medications including the name, dosage, and frequency taken OR ask your Pharmacist for a copy of your medication list.
  • New patient paperwork which must be completed. 

 

 

  • Once it has been determined that you have a qualifying condition, the physician will input your information into the Office of Compassionate use Registry.
  • The Office of Compassionate use Registry is used to establish that your are a Medical Marijuana patient.
  • Once the physician has entered your information into the Office of Compassionate use Registry your patient ID number will be generated. 
  • Your patient ID number will be provided to you at the end of your appointment.

 

Our staff provides thorough patient education in order to help our patients make an informed decision about the choice to use medical cannabis as an alternative treatment to and/or in conjunction with pharmaceutical drugs.

Dr. Brannon has been certified to conduct patient consultations for the use of medical marijuana and is licensed by the State of Florida.

He conducts every evaluation in full compliance with Florida’s Medical Cannabis Certification guidelines.
